Resolving Sys is undefined: AJAX 1.0 RTM

I was getting the infamous "Sys is undefined" error with the AJAX 1.0 RTM release. After a few days of digging I managed to track it down. This may not help every "undefined" issue but it could help you diagnose your problem too.
