Working around the differences in behavior of master pages in Visual S(1bit.us)

Microsoft Visual Studio 2008 comes with an annoying difference in handling Web Application Projects versus folder-based Websites. The difference is in the user interface for creating new content page based on a master page. The UI is neither obvious nor consistent so here I will outline the approach to work around it. To my surprise the issue did not get resolved in the recent Service Pack 1 release of VS 2008 but hopefully the Visual Studio team at Microsoft will take it whole-heartedly and fix it in the next release.
